Partners in Pediatrics

A Patient-Centered Medical Home is not a building, place or hospital. Rather, it is an approach to providing comprehensive healthcare. As your PCMH, we want to be the first place you think of for all of your child’s medical needs. We believe that the partnership between the patient, the patient’s family, and his/her primary care provider and care team is important. The care team, led by your primary care provider, is committed to providing the best healthcare services possible. Your child’s care team will work with you and any outside providers or facilities to coordinate care across all settings, including behavioral health. The team will offer appropriate medical advice, information, and self-management support based on current recommendations and evidence-based guidelines.

Your Care Team

• Knows the health history of your child
• Listens to your questions, concerns and feelings, as well as those of your child
• Learns about you, your family, your life situation, your health goals and preferences
• Coordinates your child’s care with other healthcare providers
• Creates a trusting, collaborative relationship with you and your child

You and Your Child

• Are comfortable sharing concerns and questions with your pediatric team and other health care providers
• Provide a complete and accurate medical and family history to your pediatric team to ensure that the practice has all of the most important and relevant medical information to effectively care for your child
• Routinely communicate your child’s needs and family priorities to your pediatric team
• Remember to tell your pediatric team about any care received between visits (including emergency room visits, urgent care clinics) so we can help coordinate your child’s care
• Ask questions if you do not understand something
• Keep scheduled appointments or call to reschedule as soon as possible
• Bring your insurance card to each appointment
• Pay copays and past-due balances at the time of service

We see you, the parent, as an important member of your child’s health care team. As your child matures, we hope he or she will become an increasingly active team member. Preparing your child to take the lead in caring for his or her own health is, ultimately, our team’s most important task.

Our Pediatricians and Nurse Practitioners all treat patients from Newborn until 21 years of age.

You can schedule an appointment by calling our office from 8:30am to 5:00pm. Well visits are scheduled three months ahead of time, so call with ample time if you need a school or camp physical.

Your newborn’s visit should take place within the first two weeks of birth. Please bring along any test or screening results from the hospital. If you are expecting a baby, congratulations! Our staff will be happy to schedule an appointment for you to visit our office and meet with one of our practitioners.

A physical exam, or “well visit” is a chance for our practitioners to review your child’s health and growth. It provides parents, children and practitioners the ability to go over any questions or concerns relating to your child’s health and development. A complete history will be taken and a complete physical examination will be done. A review and discussion of relevant immunizations and simple labs and prescriptions test will also be addressed. Our practitioners recommend all patients come for annual well visits.

We offer walk-in appointments Monday – Friday from 8:30 a.m. to 9:15 a.m. Walk-in visits are considered to be for urgent illnesses. There is no need to call to schedule. You may just arrive at the office during the designated walk-in visit times. Your child will be seen in the order of the names in the sign-in sheet at the front desk. Walk-in visits should be reserved for acute illnesses. Visits for chronic or ongoing problems, follow-up office visits for previous illnesses, immunizations and WIC visits will not be addressed during walk-in times. These visits should be scheduled with our office staff.

Please note, there are NO “Walk-Ins” on the following National Holidays: New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day.

During the times that the office is closed (before or after office hours) you may call us on the regular office phone number. We have an ongoing call coverage agreement with 2 other local pediatric practices, who cover for us certain nights of the week. Coverage after hours on Saturday and Sunday is done only by PIP practitioners.

In an acute emergency, always call 911 first. This is a difficult question to answer in detail; rather than looking through a “list” of potential emergency items, we advise that you as the parent or caretaker trust your own good judgment on what constitutes an “emergency” or an issue that needs to be clarified that day or night. If you call, we will call you back. No call is unanswered. In general, fever over 102, breathing difficulty, mental status changes, urine output changes, or a child under 3 months with possible fever would all be reasons to call us.
